HIV Case Management Services

HIV Case Management provides personal support to help people living with HIV stay healthy and connected to care. Case managers work one-on-one with clients to understand their needs, set goals, and connect them with helpful services.

Services include:

  • Help scheduling medical appointments and finding specialists
  • Help arranging transportation to appointments
  • Support with taking medications and understanding HIV care
  • Help with insurance and financial assistance programs
  • Connections to social services and community resources
  • Access to mental health and support services
  • Education about HIV, treatment, and available resources
  • Support in speaking up for your health and personal needs

Clients meet with their HIV case manager at least twice a year to talk about their goals and the support they may need. Case managers also help clients build skills to stay healthy, independent, and connected to care.

To receive case management services, you must have current Ryan White Part A or Part B eligibility through CAN Community Health or the Department of Health in Duval, St. Johns, Nassau, or Clay County.

For more information, call (904) 244-5992 during normal business hours, Monday–Friday, 8:00 AM–4:00 PM.

Community Resource Group (CRG)

People who receive Case Management Services or medical care at UF CARES are encouraged to join the Community Resource Group (CRG).

CRG gives members opportunities to learn, share ideas, and have a voice in their care. UF CARES research and clinical staff may also attend meetings. This gives patients and staff a chance to talk openly, learn from each other, and build stronger relationships.

The CRG meets on the second Wednesday of each month from 11:00 AM–12:30 PM. Meeting locations may change, so please contact our office for details.
