Referral for Pediatricians and Community Partners

As a primary health care provider and/or community agency, you are often a family’s first link to early intervention. You can help families engage in the early intervention process in two simple ways:

  1. Screen infants and toddlers for developmental delays and include the results in your referral.
  2. Refer infants and toddlers birth through 36 months (45 days from the 3rd birthday) for an evaluation to determine a child’s eligibility for our program.

When discussing early intervention supports with a family you are NOT referring their child to therapy, you are referring the family for an eligibility evaluation. According to Federal policy, services must be decided upon by the Individual Family Support Plan (IFSP). If eligible, the family will receive service in the natural environment that the child lives, learns and plays.

Please note that if a child has an Established Condition , they are automatically eligible fer services. An assessment of the child will determine the scope of services.

If a child has an at-risk condition, they are eligible for service coordination until they are 36 months and/or more services if eligible. Please see lists for full description of qualifying conditions.

You can make your referral online at www.floridaearlysteps.com or use the referral form to fax your referral to: (904) 798-4544.
