Program Director's Welcome Hospice and Palliative Medicine Fellowship Kelly C. Komatz, M.D., M.P.H., F.A.A.P., F.A.A.H.P.M. Program Director Welcome to the Hospice and Palliative Medicine Fellowship in the department of pediatrics at the University of Florida College of Medicine – Jacksonville. This program is accredited through the Accreditation Council for Graduate Medical Education, or ACGME. This fellowship program provides a robust and diverse experience in caring for adult and pediatric patients with chronic medical conditions along the care continuum from palliative care to hospice level of care. The collaborating partners include Community Hospice and Palliative Care of Northeast Florida, Wolfson Children’s Hospital, and Brooks Rehabilitation Hospital. This ACGME-approved one-year fellowship has one position with a pediatric focus. The fellows' training includes several locations unique to Jacksonville Florida including UF Health Proton Therapy Institute, Brooks Rehabilitation Hospital, community hospice providing home-based hospice care to the greater Jacksonville area for both adults and children and dedicated outpatient clinics for children and young adults with chronic medical conditions (JaxHATS and Bower Lyman Center for Medically Complex Children). Our fellowship has been active for over six years and we have a 92% board pass rate. Our program is unique from other pediatric hospice and palliative medicine due to the focus on the patient and their families in the community setting. We have 13 four-week blocks that focus on patient care outside of the hospital setting. Jacksonville has diverse outpatient pediatric clinical experiences including: The Bower Lyman Center for Medically Complex Children – a medical home for pediatric patients birth to age 21 years of age with multiple chronic medical conditions and dependence on technology Jacksonville Health and Transition Services (JaxHATS) – a transition program for young adults from 16 to 23 years of age with developmental disabilities and chronic medical conditions Physical medicine and rehabilitation Genetics If you are interested in applying to our program please submit an application through the Electronic Residency Application Service (ERAS).
