How to Apply Pediatric Hospital Medicine Fellowship Top Applicants must be board-certified or board-eligible in pediatrics upon entering the fellowship program. Applicants may access the PHM common application at http://phmfellows.org/. Please download the application, include your personal statement, an updated CV, three Letters of Recommendation (if you are a current resident, one letter must be from your current Program Director) and email to matthew.garber@jax.ufl.edu and laura.lowry@jax.ufl.edu. Division faculty review all completed applications and supporting documentation. Competitive candidates are invited for an interview, at which time both faculty members evaluate candidates. All interviewed candidates are scored by the division faculty as to academic credentials, letters of recommendation, professionalism, interpersonal skills and consistency of career plans with the goals of our program.
